                  <text>Resolution of the Board of Trustees of the Village of
Sleepy Hollow to Schedule Grievance Day
BE IT RESOLVED that the Assessor of the Village of Sleepy Hollow has prepared the Tentative
Assessment Roll for the current year, and that said Assessment Roll was filed with the Village
Clerk at her office in the Municipal Building, 28 Beekman Avenue, Sleepy Hollow, New York on
February 1, 2015, where it may be seen and examined by any persons at all times during
business hours between 8:30 AM and 4:30 PM until the 17^ day of February 2015 and that on
such day in the Board Room, Municipal Building, 28 Beekman Avenue, In said Village, between
the hours of 4 PM and 8 PM, the Board of Trustees of said Village, sitting as the Board of
Assessment Review, will meet for the purpose of hearing complaints in relation thereto, on the
application of any person considering himself aggrieved thereby.

WHEREAS, The Village Board Is desirous of retaining an architect to design a comfort
station at Sykes Park;
WHEREAS, Architect, Mondana Rezania has submitted a proposal for this project In the
amount of $11,200 (attached); and
WHEREAS, The design costs will be paid for through a New York State Office of Parks,
Recreation and Historic Preservation Grant; and
WHEREAS, The Village Board has reviewed the proposal and has found it to be
acceptable;
NOW, THEREFORE, BE IT RESOLVED that the Board of Trustees accepts a proposal
from Architect, Mondana Rezania to design a comfort station at Sykes Park In the
amount of $11,200.

T h a n k y o u for giving m e the opportunity to submit my proposal. A s per our meeting on Jan 09,
2015, t h e following will delineate m y services.
1.0 P R O J E C T DESCRIPTION
Construction of a comfort station that houses two single use accessible toilets (one male a n d o n e
female) a n d a storage r o o m for park's use. Building size is estimated to b e 15' by 25'. Two areas
on the park are being c o n s i d e r e d for comfort station location. O n e location is on t h e north east
corner of t h e park and t h e other o n e is on the north side of the park. Client has indicated that t h e
comfort station in Legion Park located in Elmsford can be used as a model.
2.0 S C O P E O F S E R V I C E S
A. Preliminary Phase

Client shall provide a survey to Architect.
Based on t h e P R O J E C T D E S C R I P T I O N Architect shall d e s c r i b e basic overall design
parameters w h i c h will be s h a r e d with Client for c o m m e n t a n d approval.

B. Schematic Design Phase
Architect shall prepare schematic/preliminary floor plan and elevations for the project. Architect
shall meet with Client to review design drawings to obtain f e e d b a c k , a n d m a k e any necessary
changes. This phase m a y n e e d to be repeated until Client a p p r o v e s t h e design.
0. Final Design Phase
Architect shall prepare final floor plans and elevations for the n e w work. Meet with Client to review
drawings a n d m a k e any necessary changes.
D. Construction Documents Phase
Architect shall prepare construction documentation consisting of d r a w i n g s a n d specifications that
will be suitable for filing with t h e Building Department and for construction by a qualified General
Contractor. Drawings shall include:
Engineering Note: S c o p e of Services shall n M include engineering w o r k for: mechanical,
electrical, plumbing, a n d fire suppression (sprinkler). Architect shall assist Client in obtaining t h e
services of such consultants for the project.
E. Filing with Department of Building
File construction d o c u m e n t s with building department. Revise drawings to incorporate any review
comments by Building D e p a r t m e n t a n d resubmit drawings. Obtain approval from building
department.

G. Construction Contract Administration.
Architect shall make site visits to monitor the progress of the work at regular intervals to
determine whether the work is in accordance with the construction documents, and shall
recommend rejection of the work that is not. Architect shall review general contractor's request
for payments and will generally keep Client Informed of the status of the project and will guard
against deficiencies In the work.

3.0 COMPENSATION
A lump sum fee of $11,200. Payment schedule shall be:

$200 upon signing of this contract agreement. This initial payment shall
be credited to final invoice.

4.0 EXCULSIONS
1. This flat fee does not include any work not specified in this agreement. Any work not
specified in this agreement is considered Additional Services. The fee for performing
Additional Services is $100 per hour or a mutually agreed lump sum fee.
2. Client shall pay for any out of pocket expenses such as cost of prints, filing fees,
messenger service, and postage.

SCOPE OF WORK
1. Contract Documents
Preparation of contract documents for the engineering services for the
comfort station.
Any environmental removals, asbestos, lead paint, oil contamination will be
performed by others.
2. HVAC.

b. No heating or air conditioning.
3. Plumbing
a. Provide water distribution and sanitary piping as required.
b. Provide secondary backflow prevention device.
c. No hot water piping or equipment.

4. No site lighting.
5. Engineering of site services are not included.
6. Reuse existing utilities.

1. Prepare required documents.
2. Prepare the design concepts, sizing all pertinent components required for the
installations.
3. Prepare the final drawings with specifications on drawings.

ARRANGEMENT OF FEE
1. The preceding shall be furnished for a lump sum fee of Four Thousand
Eight Hundred Dollars and No Cents ($4,800.00). It is anticipated
this project will be done on CAD. Any additional work to be on a time and
material basis. Partial payments will be allowed.
2. Invoices to be billed monthly and are due within 20 days. After 30 days they
will be subject to a 5% late fee.
3. Normal expenses will be included in the lump sum fee. Billable expenses
include filing fees, computer plots, prints, messenger service, etc. if requested.
4. Additional work, field meetings/trips will be in addition to the lump sum fee
and shall be billed at employee's standard hourly rates, per approval of owner
and contract amended accordingly.

�Resolution to Adopt Local Law Number 2,2015,
Restricting Parking on the north side of College Avenue
by amending Village Code Section 400-7
Whereas, the Board of Trustees (BOE) held a public hearing on January 13,
2015 to hear and consider restricting parlcing on the north side of College Avenue in
front of the church located at the intersection of College Avenue and Route 9; and
Whereas, there were no objections to said restriction for this location;
Now, therefore be it resolved, the BOE hereby adopts Local Law Number2
of 2015 which amends Village Code 400-7 to include restricted parking on the north
side of College Avenue, from the hours of 7 am to 9 am, all days, at a distance
commencing 78 ft from Rt 9 and ending at a distance of 118 feet from Route 9.

Whereas, Emanuel Anjo, by the attached letter dated January 31, 2015 and effective

2015, submitted his resignation letter from the Sleepy Hollow Police
Department Parking Enforcement Unit.
Whereas, said resignation letter must be formally accepted by the Village of Sleepy
Hollow Board of Trustees, and
Whereas, the Village, by resolution number 07/138/03 dated July 15, 2003, adopted a
policy for employee termination and rehire, and
Now, Therefore, Be it Resolved, that the Village accepts the resignation of
Emanuel Anjo by the attached letter dated January 31, 2015 and effective February 4,
2015, and finds that the termlnation/rehire policy of July 15, 2003 applies, and
Be it Further Resolved, that a copy of the policy be forwarded to and a copy be placed

in his personnel record with proof of service.
